Reassessment of Your Needs
If you already have an NHS wheelchair, we hope that you find your wheelchair meets your needs but we are all individual and our needs can change. Please see the following information on how and when to contact us.
Requesting information in an alternative format
To request information or any of our key documents is an alternative format such as braille, easy to read, larger print, audio or other format, please email us at enquiries@rosscare.co.uk quoting the publications title plus the format you require.
Requesting a Reassessment
If you are already known the Wheelchair Service, you can request a reassessment at any time. The only exception is for requests for powered wheelchairs, and we will require a referral from a health professional.
For more information on powered wheelchair requests, please read the frequently asked questions below. You can request to be placed on the waiting list for a review by calling your local customer services team and we will send you a request form to complete as we need to ensure we have up to date information in order to allocate your request appropriately.
If you experience any of the following, please request a reassessment:
- Redness of your skin that does not fade within one hour of being out of your wheelchair. Please contact your GP and the wheelchair service if you have blistered or broken skin and please consult any guidance you have been given by healthcare professionals.
- Medical changes since your last appointment that affect your ability to use your wheelchair or go about day-to-day life.
- Changes to your posture or weight that are affecting any pressure areas, your posture, or safe use of your wheelchair.
- Your wheelchair is broken or unsafe to use.
Closing Your Referral
You will have received a handover document and conditions of loan, please keep this information safe and refer to it for any specific information on how to use your wheelchair and how to contact us for repairs.
Your referral will be closed but we do not discharge you from the service.
WATCh Outcomes: Follow-up
You may recall that we asked you to complete a WATCh form asking you what you wanted to achieve with the wheelchair (or buggy). We will contact you in about 8-12 weeks time to find out how the wheelchair is working for you.
A copy of your form has been given to you. Please help us understand if we are meeting your needs and complete the form. We will contact you by phone or email, if we have these details on your record. Gaining this information is helpful for us to improve and develop the service that we offer.
Feedback
If you want to leave us any feedback, we are always pleased to hear about your experience or suggestions as all feedback is used to continually improve the service.
Your voice is essential to how we run our services and there are a number of ways you can provide your feedback and input.
Visit our Feedback page to learn more and to send us your feedback.
Video Wheelchair Guides
We have created a series of videos to help Service Users, including information on pressure ulcer prevention and care, positioning in your wheelchair, and a guide to children's seating. We also highly recommend watching a series of informative short videos created by Leeds Teaching Hospitals NHS Trust. These videos are a great introduction to your NHS wheelchair, including how to operate it safely and keep it running well.
