The information on this page is intended to help Healthcare Professionals refer new clients to us. We can only accept referrals from appropriately qualified Healthcare Professionals.

Establishing Eligibility
Commissioners, under guidance from NHS England, have created eligibility criteria for Wheelchair Services across the UK. The eligibility criteria may differ depending on Wheelchair Service/location. If your service user requires a short term loan of a wheelchair, please contact the Red Cross (other providers may be available).

If you are a Healthcare Professional and want to refer to our service, please complete a referral form for the Wheelchair Service you would like to refer into. Referrals must meet the eligibility criteria to be accepted, so please read them carefully to make sure your client is eligible for the service.

Provision will be considered for those individuals within residential and nursing care homes who meet the general criteria and for whom the wheelchair will provide a means of independent mobility.

Healthcare Professionals Guide to Referring an Adult for a Powered Wheelchair
This guide is designed to support you to make a clinical decision as to whether the individual meets the criteria for provision of an NHS powered wheelchair from the Wheelchair Service.
If your Wheelchair Service is not listed, please contact your local Customer Services team directly.
Joint Appointments
We welcome the opportunity to be involved in supporting a holistic care plan, working with a service user's Multidisciplinary Team (MDT). If you would like to work together, please provide contact details where indicated on the referral form. Providing details of the availability of the MDT members by email when returning your form is very helpful.
Assessments normally need to be carried out at our clinics, so that we have direct access to specialist equipment and advice.
Please note that we require your client’s consent to invite you or any other professionals to any scheduled appointment.


Feedback
We want the wheelchair and repair services we provide to be excellent for all of our stakeholders. We believe you should be able to refer your service users to us quickly and easily.
You can provide feedback to any staff member at any time. If you have not received the level of service you expect from us, we want you to tell us as soon as possible. By doing so, you can help us get to the bottom of the problem quickly and effectively.
We take your concerns and complaints very seriously, and they are always treated in the strictest of confidence. We will investigate your concerns as quickly as possible and work with you to find an appropriate solution.
